Italiano I hereby grant permission to the University of Washington (UW) and its assigns and licensees to take photographs or videos of me, and to make recordings of my voice. I give the UW permission to use these images, videos, and recordings, as well as my likeness, name, and voice, as follows:
- The use may include reproduction, distribution, derivative works, display, and performance.
- The use may be in composite or modified forms and in any media, now known or later developed, including without limitation newspapers, television, radio, the World Wide Web, and social media.
- The use may be for any purpose throughout the world and in perpetuity, including, without limitation, education, trade, advertising, and promotion.
I further acknowledge that I will not be compensated for these uses, and that the UW exclusively owns all rights to the images, videos, and recordings, and to any derivative works created from them. I waive the right to inspect or approve of the uses of any printed or electronic copy. I hereby release the UW and its assigns and licensees from any claims that may arise from these uses, including without limitation claims of defamation or invasion of privacy, or of infringement of moral rights or rights of publicity or copyright.
This Release is binding on me, my heirs, assigns, and estate. The UW is not obligated to use any of the rights granted under this Release. This Release expresses the complete understanding of the parties.

THE FOLLOWING AGREEMENT IS BETWEEN THE CARBON LEADERSHIP FORUM (CLF) AND THE SPEAKER LISTED ABOVE (SPEAKER).
PLEASE REVIEW THIS AGREEMENT AND CLICK “SIGN THE AGREEMENT” TO ACCEPT.
A) Webinar Registration
All SPEAKERS (presenters, moderators, and facilitators) are required to register for any CLF Webinar.
B) Session Details
Speaker agrees to prepare, present and discuss during the speaking session listed above whether live online or pre-recorded. *Final date and time of your presentation are subject to change.
C) Audio Visual Equipment
Speakers must use their own equipment to access the online platform and present their content. Speakers must have personal access to internet capabilities with a minimum speed of 10Mbps (upload and download). Test your internet speed here.
D) Presentation Materials
All session groups must provide an electronic copy of their presentation (PDF preferred) a week before the scheduled webinar, or later by agreement with the CLF moderator.
TERMS AND CONDITIONS: WAIVER
The undersigned agrees and acknowledges the Agreement with the CLF with regard to participating as a SPEAKER at a CLF Webinar.
A) Presentation
SPEAKER agrees to present at the conference and to adhere to the content and intent of heir talk as previously determined with the CLF.
B) Material Reproduction
SPEAKER grants the CLF the necessary permissions and rights to post their picture, bio, title, abstract and presentation recording on INSTITUTE website, program collateral, and distributed materials.
C) Recording Waiver
SPEAKER grants the CLF the necessary permissions and rights to transcribe, reproduce, and distribute video, photo or recordings of the SPEAKER and presentation in complete or partial form for the purposes of the CLF, provided that the original intent and content is not substantially altered. SPEAKER authorized the use of their name in connection with these recordings, and the use, reproduction, and exhibition of their likeness and voice to be used for any purpose that CLF deems appropriate. SPEAKER agrees to release CLF from all claims or damages arising from or related to the use of these recordings.
RISK
SPEAKER agrees to indemnify, hold harmless, and defend the CLF for any alleged liability resulting from SPEAKER’s presentation, both verbal and written, including libel, slander, violation of copyright, or misstatement of fact. In addition, SPEAKER shall not use copyrighter music in the presentation without obtaining prior written permission from the CLF.
TERMINATION
This Agreement will be binding on both the SPEAKER and the CLF, and there shall be no right of termination on the part of the SPEAKER for the purpose of accepting a different engagement on the same date or efforts to book an alternate SPEAKER for the CLF engagement.
The performance of this Agreement by either party is subject to acts of God, war, government regulation, disaster, fire, strikes, civil disorder, inclement weather, flight delays or curtailment of transportation facilities unreasonable delaying attendees from attending, illness or injury, or similar cause beyond the control of the parties making it illegal or commercially impossible to uphold the meeting/convention or to make the presentation. This agreement may be terminated upon the breach of any material term of this Agreement, provided written notice of such termination is given.
DEFAULT AND FAILURE TO PERFORM
In the event of any failure or refusal of SPEAKER to perform obligations under this Agreement, all fees and expenses will be considered null and void, and the CLF (except in cases beyond either parties’ control as outlined above – SEE TERMINATION) will not be liable for any associated speaking fees. Any deposit(s) or payments associated with this Agreement or previously paid by the INSTITUTE in accordance with this Agreement will be refunded to the CLF by SPEAKER within 30 days of default or termination of this Agreement.
INDEMNIFICATION
Each party agrees to indemnify, defend and hold harmless the other, their officers, directors, partners, agents and employees from any and all claims, loss, damage or expense (including attorney’s fees) arising from the negligence or willful misconduct of the indemnifying party, its agents and employees in the performance of its duties and responsibilities under this Agreement.
LAW AND VENUE
This agreement contains all of the terms agreed upon by the parties with respect to the subject matter of this agreement and supersedes all prior agreements, arrangements, and communications between that parties concerning such subject matter. This agreement shall be binding upon execution by both parties and inure to the benefit of both parties and their successors and assigns. This Agreement may not be altered by either party without the express written consent of both parties. All representations made in this agreement shall survive the termination of this Agreement.
ACCEPTANCE
To accept this agreement, the SPEAKER must complete all sections of this Agreement and sign this electronic form no later than one week prior to the scheduled webinar.
TERMS AND CONDITIONS: PROGRAM POLICIES
**Our goal is to have at least 51% female representation among out speakers and presenters.
**Our goal is to support ethnic and racial diversity and inclusiveness in our program.
**Speakers who subsequently are unable to attend the conference must notify the Institute’s staff in writing as soon as possible to [email protected]. Replacements must be approved by the CLF, which also reserves the right to modify or cancel any session based on a change in presenters / moderators / facilitators.
**If a presenter’s / moderator’s / facilitator’s professional affiliation changes, the CLF’s staff must be notified immediately. The CLF reserves the right to change or remove presenters / moderators / facilitators based on change of organizational or professional affiliation.
**The CLF may modify any submitted session by adding or removing speakers or requesting content changes or additions.
**No sessions based upon an advertisement or product will be accepted into the program.
**Exceptions to these policies may be made at the discretion of the CLF. Please contact us at [email protected] with questions around policies.
